JUNGLE YELLOW FEVER C olombia. — Two young male patients were admitted to the Casanare Hospital in Yopal City, Casanare Intendency, on 25 May 1976, with symptoms of jaundice, vomiting and fever o f two days duration. The initial clinical impression was infectious hepatitis. Two days later the patients had bloody vomitus and stools, and died with anuria and signs of severe brain involvement. On 28 May, three new patients were admitted with the same symptoms. The epidemiology unit of the Ministry o f Health then became suspicious that there was a jungle yellow fever outbreak. The diagnosis of yellow fever was confirmed by pathological examination o f liver samples and serological tests at the National Institute of Health.
FIÈVRE JAUNE DE BROUSSE C olombie . — Le 25 mai 1976, deux jeunes malades du sexe masculin étaient admis à l’Hôpital Casanare dans la ville de Yopal, district de Casanare; ils présentaient des symptômes d ’ictère, avec vomissements et fièvre remontant à deux jours. Le tableau clinique conduisit d ’abord à penser qu’il s’agissait d ’hépatite infectieuse. Deux jours plus tard, les malades vomissaient du sang et avaient des selles sanglantes puis décédaient d ’anurie avec accompagnement de signes graves d ’atteinte cérébrale. Le 28 mai, trois autres malades étaient admis avec les mêmes symptômes. La Direction de l'Epi démiologie du Ministère de la Santé commença alors à craindre une poussée de fièvre jaune de brousse. Le diagnostic de fièvre jaune fut confirmé par l’examen anatomo-pathologique d ’échantillons de foie et par des épreuves sérologiques pratiquées à l’Institut national de la Santé. Au cours de la poussée épidémique, le diagnostic de fièvre jaune de brousse fut porté chez 35 malades, sur la foi de critères épidémio logiques, cliniques et sérologiques. Onze cas de fièvre jaune à issue fatale furent confirmés par les examens histopathologiques. Le dernier cas de fièvre jaune de brousse dans le district de Casanare remonte à 1956, où il fut constaté dans ia localité de Ten, munici palité de Tamara. On a noté une association des cas de fièvre jaune avec la profession de bûcheron. On en a conclu à la nécessité d'orien ter les activités de surveillance, de vaccination et d ’éducation vers ce groupe de population. Devant la situation d ’urgence créée par la poussée épidémique du Casanare, une campagne de vaccination de masse a été lancée. On a eu recours au vaccin anti-amaril 17D fabriqué par l’Institut national de la Santé et l'on a ainsi protégé environ 90% de la population exposée.
During the outbreak, 35 patients were diagnosed with jungle yellow fever on the basis of epidemiological, clinical and serological criteria. Eleven fatal cases were confirmed by histopathological examination. The last case of jungle yellow fever in Casanare had occurred in 1956 in the locality of Ten, Municipio of Tamara. An association of yellow fever cases with the occupation of wood cutter was found. This observation indicated that the activities of surveillance, vaccination and education should be directed to this gtoup of the papulation. Due to the health emergency caused by the Casanare outbreak, a massive vaccination campaign with yellow fever 17D vaccine, produced by the National Institute o f Health, was carried out, covering about 90% of the population at risk.
