Meningococcal disease, serogroup W135 During 2000, there was an international outbreak of meningococcal disease caused by an unusual serogroup, W135.1 As with all types of meningococcal disease, early diagnosis and treatment are essential. The symptoms of group W135 meningococcal disease are the same as for other groups of the disease: sudden onset of intense headache, high fever, nausea and vomiting, photophobia and stiff neck. WHO recommends that chemoprophylaxis be given to close contacts of the cases, such as persons sleeping in the same dwelling. In most countries rifampicin is recommended. In preparation for the Umra and Hajj seasons for 2002, the Ministry of Health of Saudi Arabia has notified the ministries of health of all countries from which pilgrims arrive, that vaccination against meningococcal meningitis with the quadrivalent vaccine (serogroups A, C, Y and W135) has been added to the health requirements for arrivals coming to the Umra and Hajj. WHO encourages national reference laboratories to closely monitor meningococcal disease. In 2001 the following countries have reported to WHO cases of W135 meningococcal disease, associated with international travel or contact with travellers to Saudi Arabia. Burkina Faso. 4 cases have been reported. Neisseria meningitidis serogroup W135 has been laboratory-confirmed. 1

Méningococcie, sérogroupe W135 Il y a eu en 2000 une flambée de méningococcie de portée internationale, due à un sérogroupe peu commun, W135.1 Comme pour tous types de méningococcie, il est essentiel de faire un diagnostic rapide et de traiter les cas immédiatement. Les symptômes de méningococcie de groupe W135 sont les mêmes que pour les autres groupes de la maladie: céphalées intenses subites, forte fièvre, nausée et vomissements, photophobie et raideur de la nuque. L’OMS recommande qu’une chimioprophylaxie soit donnée aux proches contacts des malades, par exemple les personnes dormant sous le même toît. Dans la plupart des pays, la rifampicine est recommandée. En prévision des saisons d’Umra et de Hadj, le Ministère de la santé d’Arabie saoudite a notifié les ministères de la santé de tous les pays dont sont originaires les pèlerins que la vaccination contre la méningite à méningocoques par le vaccin quadrivalent (sérogroupes A, C, Y et W135) a été ajoutée aux dispositions sanitaires requises pour les voyageurs se rendant à l’Umra et au Hadj. L’OMS encourage les laboratoires de référence nationaux à surveiller de près la méningococcie. En 2001, les pays suivants ont signalé à l’OMS des cas de méningococcie de groupe W135, associés à des voyages internationaux ou au contact avec des personnes s’étant rendues en Arabie saoudite. Central African Republic. 3 cases (Hajj pilgrims) have been reported. N. meningitidis serogroup W135 has been laboratory-confirmed. France. 2 cases (close contacts with Hajj pilgrims) and no deaths have been reported. N. meningitidis serogroup W135 has been laboratory-confirmed. Norway. 4 cases (2 contacts with Hajj pilgrims) and no deaths have been reported. N. meningitidis serogroup W135 has been laboratory-confirmed. Saudi Arabia. 109 cases (predominantly Hajj pilgrims from outside Saudi Arabia) including 35 deaths reported between 9 February and 22 March 2001. N. meningitidis serogroup W135 has been laboratory-confirmed in more than half of the cases. Singapore. 4 cases (3 close contacts with Hajj pilgrims, 1 history of travel to Saudi Arabia) and no deaths have been reported. Two of the cases occurred in January 2001, before the main period of pilgrimage to Saudi Arabia. N. meningitidis serogroup W135 has been laboratory-confirmed. United Kingdom. 28 laboratory-confirmed cases, including 7 deaths have been reported. Of these cases, 8 were pilgrims returning from the Hajj, 9 were close contacts of the latter, and data are outstanding on the remaining cases. Two cases occurred in January 2001 prior to the main pilgrimage period. N. meningitidis serogroup W135 has been laboratory-confirmed. Ⅲ

Smallpox eradication: temporary retention of variola virus stocks In May 1999 the World Health Assembly by resolution WHA52.10 authorized temporary retention up to, but not later than, 2002 of the existing stocks of variola virus at the current locations,1 for the purpose of further international research. The Assembly requested the Director-General to appoint a new group of experts to establish what research, if any, must be carried out in order to obtain consensus on the timing for the destruction of the existing variola virus stocks. In accordance with this resolution, a new group of experts was appointed; this WHO Advisory Committee on Variola Virus Research is composed of 16 members from all WHO regions. At its first meeting (Geneva, 6-9 December 1999), attended also by 10 advisers from fundamental applied research and regulatory agencies, the Committee agreed that a scientific subcommittee be established for the purpose of overseeing future research on variola virus, with members of this subcommittee to be drawn from the Advisory Committee on Variola Virus Research.

Eradication de la variole: maintien temporaire des stocks de virus variolique En mai 1999, l’Assemblée mondiale de la Santé a décidé, dans sa résolution WHA52.10, d’autoriser le maintien temporaire, mais au plus tard jusqu’en 2002, des stocks existants de virus variolique dans les sites actuels 1 pour permettre la poursuite des travaux de recherche internationaux. L’Assemblée a prié le Directeur général de nommer un nouveau groupe d’experts chargé de décider des recherches à effectuer, le cas échéant, pour parvenir à un consensus sur la date de destruction des stocks existants de virus variolique. Conformément à cette résolution, un nouveau groupe d’experts – le Comité consultatif OMS de la recherche sur le virus variolique – composé de 16 membres de différents pays représentant toutes les régions de l’OMS, a été désigné. A sa première réunion (Genève, 6-9 décembre 1999), à laquelle ont également assisté 10 conseillers représentant des instituts de recherche fondamentale et appliquée et des organismes de réglementation, le Comité est convenu de la nécessité d’établir un sous-comité scientifique chargé de superviser la recherche future sur le virus variolique, et dont les membres feraient partie du Comité consultatif OMS de la recherche sur le virus variolique.

State of strain collections and viability of viral isolates The Centers for Disease Control and Prevention hold 451 viral isolates derived from several different national collections. Most are isolates of variola virus and a database has been created to link them with available diagnostic and epidemiological data. Of 49 strains, selected on the basis of geography, year of isolation and low passage history, that have been further analysed, 45 were shown to be viable. These isolates came from Asia (21), Africa (16), Europe (5), South America (2) and North America (1). Many showed uniform plaque morphology and grew to high titre in tissue culture. Of the 45 viable isolates, 37 came from in vitro cultured material and the remainder from scab (nonpassaged) samples. The samples currently held at the Russian Centre, VECTOR, began to be collected in Moscow in the mid1950s. The collection was augmented with isolates obtained by the then WHO collaborating centre on smallpox in Moscow during the diagnostic studies that supported the smallpox eradication programme. The present collection includes primary material (scabs), frozen liquid cultures and lyophilized samples. Not all samples have been tested for viability; 5 primary scab isolates, 4 of 9 frozen cultures and all 6 lyophilized strains have demonstrable viability. Cooperation between staff at the 2 collaborating centres has begun, in order to ensure that any future work on viral characterization, including the transfer of reagents, is adequately coordinated. The Committee concluded that additional work may be needed to assess the viability of the stocks held in VECTOR, and that further molecular characterization of additional strains may be valuable in helping to identify strains from which further DNA sequences could be determined.

Etat des collections de souches et viabilité des isolats viraux Les Centers for Disease Control and Prevention détiennent 451 isolats viraux dérivés de différentes collections nationales. La plupart sont des isolats de virus variolique et une base de données a été créée afin de les relier aux données diagnostiques et épidémiologiques disponibles. Sur 49 souches soumises à une analyse plus poussée et choisies en fonction de leur origine géographique, de leur année d’isolement et de leur faible nombre de passages, 45 ont été trouvées viables. Ces isolats provenaient d’Asie (21), d’Afrique (16), d’Europe (5), d’Amérique du Sud (2) et d’Amérique du Nord (1). Nombre d’entre eux présentaient une morphologie des plages uniforme et atteignaient un titre élevé en culture tissulaire. Sur les 45 isolats viables, 37 provenaient d’un matériel cultivé in vitro et les autres de prélèvements de croûtes cicatricielles n’ayant subi aucun passage. Les échantillons actuellement détenus au Centre russe VECTOR ont commencé à être rassemblés à Moscou vers le milieu des années 50. Cette collection a été complétée par des isolats obtenus, au cours des études diagnostiques réalisées dans le cadre du programme d’éradication de la variole, par le laboratoire qui était à l’époque le centre collaborateur de l’OMS pour la variole à Moscou. La collection actuelle comprend du matériel primaire (croûtes cicatricielles), des cultures conservées à l’état congelé et des échantillons lyophilisés. Tous les échantillons n’ont pas fait l’objet d’une épreuve de viabilité; 5 isolats primaires, 4 des 9 cultures congelées et l’ensemble des 6 souches lyophilisées présentent une viabilité démontrable. Une coopération entre le personnel des 2 centres collaborateurs a débuté dans le but d’assurer une coordination convenable de tous les travaux futurs sur la caractérisation du virus, y compris le transfert des réactifs. Le Comité a conclu qu’il pouvait être nécessaire d’entreprendre des travaux supplémentaires pour évaluer la viabilité des stocks détenus au Centre VECTOR, et qu’une caractérisation moléculaire plus poussée de nouvelles souches pouvait être utile pour aider à identifier les souches pour lesquelles de nouvelles séquences d’ADN pourraient être déterminées.

Phylogenetic analysis with DNA amplification technologies Several techniques based on the polymerase chain reaction (PCR) to facilitate the characterization and phylogenetic analysis of variola virus isolates were described. These included restriction fragment length polymorphism of PCR products amplified with a variety of primers, and multiplex PCR analysis. sequences in the central conserved genomic region were used for the comparison of all orthopoxviruses whereas those complementary to sequences located towards the genomic termini were used to provide species- and strainspecific data. The Committee concluded that significant progress had been made in applying PCR technology to the investigation of phylogenetic relationships between the orthopoxviruses, particularly variola viruses.

Detection and differentiation of orthopoxvirus DNA Several methods using DNA amplification technologies were described for the detection and subsequent diagnosis of orthopoxvirus infections. A major objective of this work is the real-time identification of smallpox viruses. The basic procedures used are similar to those already described for phylogenetic analysis of different variola virus isolates. The detection and differentiation of orthopoxvirus strains and individual strains of variola virus generally involve the generation of PCR-amplified products from both conserved and variable regions of the genome. Different groups have developed different platforms for the process of detecting amplified DNA products. The Committee noted the enormous progress in this area. Although a major limitation to these procedures was the methods used to obtain the initial DNA samples, some reliable and rapid procedures with commercially available reagents are becoming available. The Committee also noted that the specificity of the procedures completely depended on the sequences of the primers used for amplification. The detection of nucleotide sequences in cowpox virus that were previously considered to be specific to variola virus, emphasized the point that the use of a single locus for PCR amplification is insufficient to provide unambiguous identification. Members of the Committee questioned the need for rapid analytical procedures that were sufficiently sensitive to differentiate between variola subspecies when the clinical management of infected individuals would be the same. It was recognized that the ability to detect the presence or absence of any orthopoxvirus in real time would be needed in emergency public health situations.

Détection et différenciation de l’ADN d’orthopoxvirus Plusieurs méthodes faisant appel aux technologies d’amplification de l’ADN ont été décrites pour la détection et le diagnostic ultérieur des infections à orthopoxvirus. L’un des objectifs majeurs de ces travaux est l’identification des virus varioliques en temps réel. Les procédures de base sont similaires à celles déjà décrites pour l’analyse phylogénétique des différents isolats de virus variolique. La détection et la différenciation des souches d’orthopoxvirus et des souches individuelles de virus variolique comportent généralement l’obtention de produits amplifiés par PCR à partir des régions conservées et variables du génome. Différents groupes ont élaboré des plates-formes pour les procédures de détection de l’ADN amplifié. Le Comité a pris note des progrès énormes réalisés dans ce domaine. Bien que ces procédures soient considérablement limitées par les méthodes utilisées à l’origine pour obtenir les échantillons d’ADN, des procédures fiables et rapides faisant appel à des réactifs du commerce deviennent maintenant utilisables. Le Comité a également noté que la spécificité des procédures dépendait entièrement de la séquence des amorces utilisées pour l’amplification. Nucleotide sequence analysis of variola virus DNA The Committee was informed that the nucleotide sequences of 3 complete variola virus genomes were now available. Substantial parts of the genomes of 3 other variola virus strains – Congo 70, Somalia 77 and India 7124 – had also been determined. The full sequence of camelpox virus, the closest known relative of variola virus, was described. Many sequence data are also available for individual genes of various other orthopoxviruses. The data obtained so far have confirmed the suspected evolutionary relationships between orthopoxviruses and have facilitated the further classification of different variola isolates into subspecies. At least 6 complete variola virus genome sequences should be available by the end of 2002. It may also be possible to obtain sequence information from scab material that has not been passaged in vitro. The Committee concluded that very good progress had been made in analysing the sequences of smallpox virus genomes. 144

Serological detection of variola virus Work using monoclonal and polyclonal antibodies against vaccinia virus proteins in enzyme-linked immunosorbent assays to detect orthopoxviruses was described. With these reagents, such tests can detect various orthopoxviruses, including camelpox, cowpox, monkeypox, vaccinia and variola viruses. The relative sensitivity for detecting the different viruses varied, and strain differentiation was not possible with this method. Work is in progress to determine whether tests using these reagents can differentiate live variola major and minor strains. The Committee concluded that much useful work was being done in this area and that it would be important for any generated monoclonal antibodies to be thoroughly characterized with respect to the variola proteins against which they react.

Détection sérologique du virus variolique Des travaux faisant appel à des anticorps monoclonaux et polyclonaux dirigés contre des protéines de virus de la vaccine, utilisés dans des tests immuno-enzymatiques de détection des orthopoxvirus, ont été décrits. A l’aide de ces réactifs, ce type de tests peut détecter divers orthopoxvirus, dont ceux du camelpox, du cowpox, du monkeypox, de la vaccine et de la variole. Antiviral agents Cidofovir inhibits a wide variety of DNA viruses including orthopoxviruses. Its mechanism of action is through selective inhibition of the viral DNA polymerase. In vitro tests for antiviral activity show that cidofovir inhibits both vaccinia and 35 different isolates of variola virus. Each isolate had similar sensitivity to the drug and induction of resistance mutations does not appear to be a problem. Other orthopoxviruses could therefore be developed as surrogate models for antiviral drug testing after variola virus stocks have been destroyed. The Committee noted that considerable progress had been made.

Animal models of smallpox Most work has been done on the aerosol infection of cynomolgus monkeys with the Yamada and Lee strains of variola virus. Clinical signs appear in infected animals by 6 days after infection. Clinical disease is apparent and the animals seroconvert but there is no mortality. The Committee concluded that this model was not suitable for assessing the efficacy of new vaccines or drugs. Further work is expected to be done with different strains of variola virus and different primate species, including a possible collaborative project with scientists at VECTOR using baboons as the animal model. The Committee concluded that progress was satisfactory but that the rate was slow. It noted the proposed work on the infection of baboons but indicated that additional work may be required to identify and characterize further surrogate models in comparison to those of variola virus infection so that a validated system for drug and vaccine assessments can be established. Ⅲ

Le Comité a conclu que ce modèle ne convenait pas à l’évaluation de l’efficacité de nouveaux vaccins ou médicaments. De nouveaux travaux devraient être réalisés avec différentes souches de virus variolique et différentes espèces de primates, éventuellement dans le cadre d’un projet en collaboration avec les scientifiques du Centre VECTOR portant sur l’utilisation du babouin comme modèle animal. Le Comité a conclu que les progrès réalisés étaient satisfaisants mais qu’ils étaient lents. Il a pris note des travaux proposés sur l’infection du babouin, mais a indiqué que des travaux supplémentaires seraient nécessaires pour identifier et caractériser d’autres modèles de remplacement par rapport au modèle d’infection par le virus variolique de façon à pouvoir établir un système validé d’évaluation des médicaments et vaccins. Influenza Brazil (5 May 2001). Since the last week of April, 5 influenza A viruses have been detected in children by indirect immunofluorescence, while 2 influenza B viruses were isolated from a child and an adult. All were hospitalized cases in São Paulo. 1
