ANTIGENIC VARIATION IN INFLUENZA A VIRUSES (Information from the World Influenza Centre, London and the International Influenza Centre for the Americas, Atlanta, dated 21 September 1972). The antigenic characterization of influenza A isolates examined between June 1971 and April 1972 is shown in Table 1. During this period only a small proportion of strains showed significant antigenic differences from prototype A/Hong Kong/1/68 (H3N2). Since May 1972 this distribution has shown considerable change. All isolates (37) from recent outbreaks of influenza in Malaysia and Singapore and current outbreaks in Australia resembled tbe variant A/England/42/72. The latter strain was initially charac terized at the Central Public Health Laboratory, London, as showing considerable antigenic “drift” from A/Hong Kong/1/68. It was isolated in both eggs and monkey kidney cultures from a case of clinical influenza in the United Kingdom in January 1972. However, subsequently a collection o f isolates obtained between July and December 1971 from outbreaks of influenza in Coonoor, South India, was found to consist predominantly of strains like A/England/42/72 and this represents the earliest isolations of the variant. Three current influenza A isolates from Australia and a recent isolate from Hawaii examined at the WHO International Influenza Centre for the Americas, Center for Disease Control (C D Q , Atlanta, Ga, USA, were confirmed as being closely related to A/England/42/72.
VARIATIONS fANTIGÉNIQUES CONCERNANT LES VIRUS GRIPPAUX A ^(Inform ations en provenance du Centre mondial de la Grippe de Londres et du Centre international de la Grippe pour les Amériques d ’Atlanta en date du 21 septembre 1972). Le Tableau 1 montre les caractères antigéniques des isolements de virus grippal A examinés entre juin 1971 et avril 1972. Pendant cette période, seule une faible proportion des souches a présenté des différences antigéniques significatives par rapport au prototype A/Hong Kong/1/68 (H3N2). Depuis mai 1972, des modifications considérables se sont cependant produites dans cette répartition. Tous les isolements (37) provenant des récentes poussées de grippe de Malaisie et de Singapour, ainsi que des poussées qui se produisent actuellement en Australie, ressemblaient à la variante A/England/ 42/72. Cette dernière, initialement mise en évidence au Laboratoire central de Santé publique de Londres, présente un « glissement » antigènique par rapport à A/Hong Kong/1/68. Elle fut isolée aussi bien sur œuf que sur culture de rein de singe dans un cas clinique de grippe qui se produisit au Royaume-Uni en janvier 1972. Cepen dant, par la suite, on découvrit qu’une collection d ’isolements effectués entre juillet et décembre 1971, lors de poussées grippales observées à Coonoor dans le sud de l’Inde, était principalement constituée de souches semblables à A/England/42/72 et qu’il s’agis sait des premiers isolements de cette variante. The results of antigenic comparisons of A/Hong Kong/1/68, A/Hong Kong/5/72 and A/England/42/72, performed with post infection ferret sera and with antisera against the purified haemagglutinm and neuraminidase antigens (where available), are shown in Tables 2 and 3. Both the haemagglutinin and neura minidase antigens of A/England/42/72 show evidence of antigenic change from A/Hong Kong/1/68. These tests indicated that both types of variants, A/England/42/72 and A/Hong Kong/5/72, con tained antigenically distinguishable haemagglutinin antigens but closely related neuraminidase antigens.

Examination of the frequency of haemagglutination-inhibitmg (HI) antibody to A/England/42/72 in recently collected sera from the United Kingdom (Table 4) shows that antibody was infrequent and at low titre. Only 17% of sera contained antibody (1:40 or greater) to A/England/42/72 whilst 67% contained antibody to prototype A/Hong Kong/1/68.

In addition, human pm- and post-vaccination sera from indi viduals receiving inactivated A/Aichi/2/68 (H3N2)1 vaccine, collected in 1968 before the A/Hong Kong/1/68 epidemics in the USA, were tested in Atlanta for haemagglutination-inhibition antibody. Although 90 % of the vaccinées had HI antibody responses of 1:40 or greater against A/Hong Kong/1/68, 50% had no detectable antibody and 90% had HI titres of 1:20 or less against A/Queensland/6/72 (a recent isolate from Australia antigenically close to A/England/42/72).
These preliminary observations strongly suggest that, should strains like A/England/42/72 become widely disseminated, the population in many areas may be largely unprotected despite previous exposure to A/Hong Kong/1/68 virus. Close considera tion should be given to the need for reformulation of current influenza vaccines. A strain of A/England/42/72 isolated in eggs is available for distribution from the World Influenza Centre, London and the International Influenza Centre for the Americas, Atlanta. A high yielding recombinant strain with haemagglutinin and neuraminidase antigens identical to those of A/England/42/72 is also available from the World Influenza Centre (strain designation: MRCI). 1 AnUgemcally similar to A/Hoag Kong/1/68 (H3N2).

AEDES AEGYPTI P o r tu g a l . — The National Health Administration has reported the absence of Aedes aegypti from the Madeira and Porto Santo Archipelago. No larval or adult stages of the mosquito were found in a survey which included the port area of the city of Fun chal, the town of Porto Santo and the airports of Santa Catarina and Porto Santo including the surrounding areas. Sp a in . — The two airports of Santa Cruz de Tenerife and Las Palmas have been reported free of Aedes aegypti. This informa tion was contained in a recent report of a comprehensive survey covering 13 localities distributed throughout the seven main islands of the archipelago and including all these localities from which A. aegypti had been reported in the past. N o larval or adult forms of A. aegypti were detected in any of these localities.

Wellington, where sporadic cases of influenza associated with virus B were reported previously, a sharp increase has been observed this week in the number of notifications of influenza-like illness. An average of six to eight cases have been examined daily by local doctors. One strain of virus A has been isolated from a patient.

Q FEVER U nited K in g d o m . — Fifty-two cases with laboratory evidence Of infection due to Coxiella burneti were reported in 1971 from laboratories in England, Wales and Ireland, approximately the same number as in previous years. The organism was isolated from blood or endocardial vegetations in three patients; the diag nosis was based on a fourfold rise in antibody titre in 32 other patients and on a single high antibody titre with supporting clinical evidence in 17.

Fifty-one reported infections were in adults, 41 in males and half of the cases were reported during the spring and early summer months in the second quarter of the year. Only seven occuired in the winter months of January, February, March and December. The geographical distribution of cases in England, Wales and Ireland does not indicate a concentration of cases in any particular areas. There were no outbreaks having occurred and no instances suggestive of case-to-case transmission. The patient’s occupation was known in 20 instances; in four cases there was an established connection with a farm and one patient was reported to have drunk unpasteurised milk on several occasions before her illness. Pneumonia or pyrexia of unknown origin were the most promi nent clinical features reported. Endocarditis was reported in seven patients and this complication was the cause of the only death reported, in a man aged 20.

Cinquante-et-un des cas signalés se sont produits chez des adultes, dont 41 du sexe masculin, la moitié des cas notifiés survenant au deuxième trimestre, au cours des derniers mois de printemps et au début d’été avec seulement sept cas pendant les mois de janvier, février, mars et décembre. La répartition géographique des cas, en Angleterre, au pays de Galles et en Irlande, ne fait pas apparaître de concentration parti culière, étant donné qu'aucune épidémie ne s’est produite et qu’au cun exemple de transmission n ’a été signalé.
