INTRODUCTION AND SUMMARY The fifty-third session of the Regional Committee for the Western Pacific was held in Kyoto, Japan, from 16 to 20 September 2002. The session was attended by representatives of Australia; Brunei Darussalam; Cambodia; China; Cook Islands; Fiji; Hong Kong (China); Japan; Kiribati; the Lao People's Democratic Republic; Macao (China); Malaysia; the Republic of the Marshall Islands; the Federated States of Micronesia; Mongolia; New Zealand; the Republic of Palau; Papua New Guinea; the Philippines; the Republic of Korea; Samoa; Singapore; Solomon Islands; Tonga; Tuvalu; Vanuatu; and Viet Nam, and by representatives of France, the United Kingdom of Great Britain and Northern Ireland and the United States of America as Member States responsible for areas in the Region. Representatives of the Joint United Nations Programme on AIDS; the Global Fund to Fight AIDS, Tuberculosis and Malaria; the Association of South-East Asian Nations; the Secretariat of the Pacific Community; an observer from Canada; the Sasakawa Memorial Health Foundation; 31 nongovernmental organizations; and three guest speakers also attended. At the opening ceremony the Minister of Health, Labour and Welfare, Japan, Dr Chikara Sakaguchi, the Governor of the Kyoto Prefecture, Mr Keiji Yamada, and the Mayor of Kyoto City, Mr Yorikane Masumoto, welcomed the members of the Regional Committee and other guests to Kyoto. On behalf of the Regional Committee, its Chairperson, Pehin Haji Abdul Aziz Umar, thanked the Government of Japan for graciously hosting the fifty-third session of the Committee. The Regional Director and the Director-General also expressed their appreciation to the Government of Japan. At the first plenary meeting, the Committee elected the following officers: Chairperson: Vice-Chairperson: Rapporteurs in English: in French: Dr Hideo Shinozaki (Japan) Dr EIiuel Pretrick (Federated States of Micronesia) Dr Tee Ah Sian (Malaysia) Dr Nao Boutta (Lao People's Democratic Republic) The Report of the Regional Director on the Work of WHO during the period 1 July 2001 to 30 June 2002 was then presented to the Committee. Comments from 4 REGIONAL COMMITTEE: FIFTY-THIRD SESSION representatives fell into the following main categories: noncommunicable diseases (particularly diabetes), tobacco control, mental health, and communicable diseases, especially outbreak surveillance and dengue haemorrhagic fever. Other issues that were raised under this item included the role of the private sector, infant and young child feeding, and the link between health status and poverty. At the second meeting, the incoming Chairperson and the Director-General addressed the Committee. Several representatives praised the raising of WHO's profile that had taken place during the term of the Director-General. Among the initiatives that had taken place during her term, they noted in particular the initiation of a process towards an international framework convention on tobacco control and the report of the Commission on Macroeconomics and Health. The Regional Director added his voice to the regrets expressed by representatives at the Director-General's decision not to stand for a second term. In her response, the Director-General observed that she had been the first woman elected to head a UN agency and told the Committee that her decision not to stand for a further term had been taken for personal reasons. The proposed programme budget for 2004-2005 was examined at the second and third meetings. Introducing the item, the Regional Director explained that there would be a more comprehensive budget presentation, including country programme budgets, at the fifty-fourth session in 2003. He also noted that, as a result of World Health Assembly resolution WHA51.31, the Region's budget had been progressively reduced over three bienniums. Commenting on the budget, several representatives expressed the view that the reduction in the regional allocation that had followed the adoption of resolution WHA51.31 should not continue beyond the end of 2005. Several also commented on the high level of anticipated extrabudgetary resources in the global document and on the allocation of these resources among the six regions and WHO Headquarters (WPRlRC53.R2). The extent to which the budget for the Region reflected global and regional priorities was also raised by a number of representatives. The Regional Director, in responding to the points that had been made, pointed out that the expenditure on global priorities under regional and intercountry activities in 2004-2005 had increased over the previous biennium. He very much regretted that some country budgets had had to be reduced, but said that, given the overall reduction for Region, some reductions in individual allocations was inevitable. He concurred with the view that the reduction in the regional allocation should cease at the end of the 2004-2005 biennium. REPORT OF THE REGIONAL COMMITTEE 5 The third meeting continued with a discussion of the fmal report on budget performance during the 2000-2001 biennium. The Committee expressed satisfaction that the budget had been implemented in full and accepted the Regional Director's report on budget performance (decision WPRlRC53(1». Introducing the next item, tuberculosis, the Regional Director said that, while a very solid foundation had been laid for the Stop TB special project, much remained to be done. In particular, he urged Member States to expand and improve the quality of directly observed treatment, short-course (DOTS) programmes. He also noted emerging issues, such as tuberculosisIHIV co-infection, drug resistance and the need to engage to private sector. All the subsequent speakers endorsed the very high priority given to tuberculosis control by the Regional Office. However, the need to increase the focus on case detection and to maximize the resources available from the Global Fund to Fight AIDS, Tuberculosis and Malaria was also stressed. The Committee noted that tuberculosis touched all countries and all social strata within countries, although it hit the poor particularly hard (WPRlRC53.RI). The fourth meeting began with a discussion of the new Global Fund to Fight AIDS, Tuberculosis and Malaria. The Regional Director described the very rapid development of the Global Fund and the work that WHO had already done to support countries from the Region to prepare proposals for the Fund. The representatives who spoke on this item were broadly supportive of both the formation of the Global Fund and the role played by WHO in helping countries to prepare proposals. Representatives noted with satisfaction the high approval rate for proposals from the Western Pacific Region. Among the concerns raised were: membership of the board of the Fund; the need to strengthen the capacity of nongovernmental organizations in its work; the threat posed to small Pacific island nations; and the need for the Fund to adopt a more comprehensive systems approach (WPRlRC53.R3). The fourth and sixth meetings continued with a discussion of sexually transmitted infections, HlV infection and AIDS. Introducing the subject, the Regional Director warned Member States against complacency; there was still a very real danger of the HIV I AIDS epidemic spreading. In particular, there was a danger of HI VIA IDS spreading from high-risk groups into the general population. In the discussion that followed, there was considerable interest in the importation of generic antiretroviral drugs and whether such importation violated the TRIPS agreement. 6 REGIONAL COMMI1TEE: FIFTY-THIRD SESSION High HIV prevalence among migrant workers, the success of 100% condom use programmes, the need to stress that higher levels of abstinence could reduce HIV I AIDS incidence, stigmatization of AIDS patients, public-private partnerships, the role of the media, and the need to change attitudes and behaviours were all raised during the debate. In response, the Regional Director noted the interest of representatives in the issue of drug availability and assured them that this issue would be covered in detail at the next session, if Member States wished to do so. Dr Peter Piot, Executive Director of the Joint United Nations Programme on AIDS, agreed with emphasis that had been placed on vigilance at the meeting. The HIV I AIDS situation in the Region would almost certainly worsen and many of those at highest risk were among the general population. A ministerial round table on Diet, physical activity and health was held during the fifth session. The round table was preceded by two presentations from invited speakers. At the beginning of the sixth meeting the moderator of the round table presented a summary of the round table's conclusions. Malaria, filariasis and other parasitic diseases were also covered during the sixth and seventh meeting. With regard to malaria, representatives of the endemic countries described the progress that had been over the last ten years and outlined some of the challenges that remained. These included climate change (which was leading to the re-emergence of malaria in some areas), the link between rural poverty and malaria prevalence, large migrant populations, and increasing resistance to antimalarial drugs, among others. With regard to filariasis control, the achievements of China were applauded, as was the commitment shown by the Governments of Pacific island countries. The deadline of 2010 for filariasis elimination in the Pacific was felt to be a challenging, but achievable, target. Representatives also described some of the activities that were taking place across tropical parts of the Region to control helminth infestations. The strong link between these interventions and other health promotion efforts in schools was stressed (WPRlRC53.R4). The seventh meeting continued with an examination of antimicrobial resistance in the Region. Representatives urged greater international attention for this issue, expressing alarm that, unless action to contain resistance was taken soon, the world could be moving into a post-antibiotics era. Various control measures were described but there was a strong consensus that much more needed to be done. The Committee agreed that this was a cross- sectoral issue that involved regulators, public health departments, prescribers, dispensers and the public (WPRlRC53.R5). REPORT OF THE REGIONAL COMMITIEE 7 The agenda item on ethical issues was introduced by Dr John Bryant, President, Council of International Organizations for Medical Sciences. Responding to the many and varied issues that were raised in the presentation, representatives were in agreement that recent progress in medical science had raised many ethical issues and that countries would benefit from guidance on how to respond to these. The Regional Director assured the Committee that the Regional Office, in collaboration with WHO Headquarters, would pursue the subject further (WPRlRC53 .R6). At the seventh and eighth meetings the Committee turned its attention to the agenda item on Essential public health functions: the role of the ministry of health. The concept of essential public health functions was generally felt to be a valuable one, and it was agreed that defining such functions would enable governments to clarify what it should be doing and to compare this with what it was actually delivering. There was, however, agreement on the need to refme the concept further and to take into account prevailing national circumstances in adapting the concept to individual countries (WPRlRC53.R7). Also at the eighth meeting, the Committee examined a number of matters covered at the Fifty-fifth World Health Assembly that had relevance for the Region. These included the resolutions that had been adopted on mental health, access to essential medicines, WHO's contribution towards the achievement of the development goals of the United Nations Millennium Declaration, and infant and young child nutrition. The Committee then selected Fiji to serve on the policy and coordination committee of the Special Programme of Research, Development and Research Training in Human Reproduction (decision WPRlRC53(2». Regarding future sessions, the Committee agreed that the fifty-fourth session would be held from 8 to 12 September 2003 at the Regional Office in Manila. The Committee also expressed appreciation to the Govemment of China for its offer to act as host to the fifty-fifth session of the Regional Committee in 2004 (WPRlRC53.R8). At the closure of the session, the Regional Director and the Committee expressed their appreciation to the Government of Japan for hosting the fifty-third session, the chairperson, vice-chairperson, rapporteurs, presenters, Rotary District 2650, and representatives of intergovernmental and nongovernmental organizations for their oral and written statements (WPRlRC53.R9). 8 REGIONAL COMMITIEE: FIFfY -THIRD SESSION II. RESOLUTIONS ADOPTED AND DECISIONS MADE WPRlRC53.RI TUBERCULOSIS PREVENTION AND CONTROL The Regional Committee, Noting that the Member States of the Western Pacific Region account for one third of the global tuberculosis burden; Recognizing that tuberculosis affects all sections of society, but particularly the poorest and most vulnerable populations and the most economically productive age group; Expressing concern that, despite the progress made in the directly observed treatment, short course (DOTS) expansion, the current case detection rate of 39% of estimated new smear-positive cases in areas of the Region implementing DOTS is still far below the regional target of70%; Expressing further concern that the quality of DOTS programmes is being compromised by the insufficient human resources assigned to tuberculosis programmes to conduct systematic monitoring and supervision; Recognizing that, in most of the countries in the Region with an intermediate burden of disease, tuberculosis notification rates have stopped declining; Further noting that the emerging challenges to tuberculosis control include multidrug resistance, tuberculosisIHIV co-infection and the low level of DOTS implementation in the private sector; Welcoming the successful foundation that has been laid by Member States and the Stop TB Special Project since the declaration of a tuberculosis crisis in the Region in 1999;1 Acknowledging the increased global and regional support being provided to tuberculosis control through such mechanisms as the Global Fund to Fight AIDS, Tuberculosis and Malaria; 1. URGES Member States: (1) to strengthen political commitment for tuberculosis control and to implement five-year Stop TB national plans for the period 200 I 2005 in order to achieve the regional targets of detecting at least 70% of smear-positive tuberculosis cases and making DOTS available to all newly detected cases by 2005; (2) to further increase fmancial resources, matching the recent increase in commitments from international partners, to expand DOTS coverage and to secure supplies of antituberculosis drugs; (3) to strengthen partnerships at the country level through Country Coordinating Mechanisms of the Global Fund, and national Interagency Coordinating Committees; I Resolution WPRJRC50.R \. REPORT OF THE REGIONAL COMMITTEE 9 (4) to increase human resources and management capacity for tuberculosis control at all levels and to enhance monitoring and supervision to improve the quality of DOTS programmes; (5) to respond to emerging challenges by (a) adopting the regional framework for the prevention, surveillance and care of tuberculosis/HIV co-infection, (b) strengthening the monitoring and management of drug-resistant tuberculosis, and (c) establishing DOTS in the private sector in collaboration with the public sector; (6) to improve the efficiency of drug management systems, including planning and monitoring, through closer collaboration between tuberculosis and drug management programmes; (7) in countries with an intermediate burden of tuberculosis, to increase the availability and quality of tuberculosis surveillance data and to identify the country- specific factors that may be preventing further declines in tuberculosis notification rates; 2. REQUESTS the Regional Director: (I) to work closely with Member States and other international organizations and partners, including the Global Fund, in order to reduce the funding gap for tuberculosis control in the Region; (2) to further strengthen technical support to countries with a high burden of tuberculosis in order to reach the regional targets of 70% case detection by 2005 and 100% of newly detected cases to be treated by DOTS; (3) to support Member States to respond effectively to emerging issues such as tuberculosislHrY co-infection, drug-resistant tuberculosis and public-private collaboration for DOTS implementation; (4) to continue to work with national interagency coordination committees, particularly in conducting reviews and comprehensive evaluations of national Stop TB programmes; (5) to support countries with an intermediate burden of tuberculosis to strengthen their surveillance systems and to analyse the country-specific factors affecting tuberculosis. Fourth meeting, 18 September 2002 WPRlRC53/SRl4 \0 REGIONAL COMMITIEE: FIFTY-THIRD SESSION WPRlRC53.R2 PROPOSED PROGRAMME BUDGET: 2004-2005 The Regional Committee, Having examined part I and part 2 of the proposed programme budget 2004-2005 to be financed from the regular budget and other sources of funds; I Noting the negative impact on collaborative programmes of the reduced regular budget allocation to the Region resulting from the implementation of resolution WHA51.31; Concerned about the current distribution of extrabudgetary resources received by the Organization among the six regions and WHO Headquarters; Noting further that the level of extrabudgetary resources for the Region has not yet been indicated in the proposed programme budget; Appreciating the presentation of the proposed programme budget; Recognizing the need to continue to monitor the impact of resolution WPRlRC50.RI on country programmes; 1. THANKS the Regional Director for his application of a results-based management approach in the preparation of the proposed programme budget; 2. URGES Member States to develop country plans that address the issues and challenges identified in part 2 and contribute to the attainment of the Organization's global objectives and goals as contained in part 1; 3. REQUESTS Members of the Executive Board from the Region: (1) to convey to the Executive Board, and through the Executive Board to the Health Assembly, the view of the Regional Committee that the reduction in the regional allocation resulting from resolution WHA51.31 should be implemented over only three bienniums, namely 2000-2001, 2002-2003 and 2004-2005; (2) to work with members of the Executive Board from other regions similarly affected, taking into account the views of the Regional Committee; 4. REQUESTS the Regional Director: (1) to transmit to the Director-General the views of the Regional Committee on the proposed global programme budget for 2004-2005, particularly with regard to the desire for more transparent criteria for deciding the allocation of extrabudgetary resources among the regions and WHO Headquarters, and the concern over reductions in some programme activities caused by the lower regional allocation; (2) to ensure that the Regional Committee's views and concerns are reflected in the guidelines for the preparation of detailed country programmes; I Document WPRlRC53/4, Annexes I and 2. REPORT OF THE REGIONAL COMMITTEE 11 (3) to present to the Regional Committee at its fifty-fourth session a proposal for the regional strategic programme budget, taking into account the further discussions at the Executive Board and the Health Assembly in 2003; (4) to develop estimates of an appropriate level of extrabudgetary resources for the Region's health needs, taking into account projected global extrabudgetary resources, and to include such funding in the budget presentation to the Regional Committee at its fifty-fourth session. WPRlRC53.R3 Sixth meeting, 19 September 2002 WPRlRC53/SRl6 THE GLOBAL FUND TO FIGHT AIDS, TUBERCULOSIS AND MALARIA The Regional Committee, Noting that the three diseases addressed by the Global Fund to Fight AIDS, Tuberculosis and Malaria not only impose a heavy disease burden on the Region and have the potential to spread rapidly, they also have a significant impact on poverty; Recognizing that the Global Fund has the potential to reduce the financial shortfall facing AIDS, tuberculosis and malaria control programmes in the Region; Commending the efforts made to establish the Global Fund within a very short period; Appreciating the special provisions made to accommodate countries with small populations and the allowances made for multicountry proposals under provisions governing small island states; Further commending the efforts made by Member States to submit high-quality proposals to the Global Fund; Noting the high approval rate of the proposals from the Western Pacific Region in the first round of proposals; Recognizing the significant role that WHO has played in supporting Member States in the Region to develop good proposals; 1. URGES Member States: (1) to further review overall AIDS, tuberculosis and malaria disease control programmes and national plans, and to analyse needs and funding shortfalls; (2) to further strengthen partnerships through close coordination with members of country coordinating mechanisms (CCMs) to ensure that the CCMs are effective in preparing comprehensive, high-quality proposals and in following-up the implementation of approved projects; 12 REGIONAL COMMIITEE: FIFTY-THIRD SESSION (3) to implement approved projects effectively and in an accountable and transparent marmer; 2. REQUESTS the Regional Director: (I) to strengthen partnerships with other agencies, such as the Joint United Nations Programme on AIDS, and regional organizations, and to ensure WHO takes an active role in the COWltry coordinating mechanisms, as part of its support for Member States; (2) to continue to improve commWlications between Member States and WHO so that accurate information on matters related to the Global Fund can be shared in a timely marmer; (3) to support countries to implement approved projects effectively and in an accountable and transparent marmer; (4) to work with Member States from the Region to facilitate their optimal representation on the Board of the Global Fund. Sixth meeting, 19 September 2002 WPRlRC53/SRJ6 WPRlRC53.R4 MALARIA, FILARIASIS AND OTHER PARASITIC DISEASES The Regional Committee, Concerned that malaria, lymphatic filariasis and soil-transmitted helminths remain formidable public health problems in the Region, affecting, especially, the poorest rural popUlations; Commending Member States and WHO for the considerable reductions in malaria morbidity and mortality during the 1 990s, for the progress made towards filariasis elimination, and for the school-based helminth control programmes that are taking place in many Member States; Noting that in most of the endemic countries in the Region morbidity and mortality data indicate that there was a reduction in the malaria burden of more than 50% between 1992 and 2000; Concerned nevertheless that malaria still causes an estimated 20 000 deaths per year in the Region and that relaxation of control efforts may lead to epidemics in endemic countries and even in cOWltries where the disease has been eradicated; Further concerned about the threat to the Region posed by multidrug-resistant fa\Ciparum malaria; Recognizing the potential for new funding for malaria programmes from the Global Fund to Fight AIDS, Tuberculosis and Malaria; REPORT OF THE REGIONAL COMMITTEE I3 Concerned that lymphatic filariasis is a debilitating and disfiguring disease that is endemic in at least 21 Member States of the Region; Noting that lymphatic filariasis can be eliminated by an effective and safe combination of drugs administered once a year for a period of four to six years; Recognizing the global goal of eliminating lymphatic filariasis by 2020; Further recognizing that the countries of the Pacific have joined together to form the Pacific Programme to Eliminate Lymphatic Filariasis (PacELF) and that PacELF has set a goal of eliminating filariasis by 20 I 0, ten years earlier than the global goal; Concerned that helminthiasis has a negative impact on children throughout the Region, stunting their growth and reducing their ability to learn and perform at school; Noting that effective anthelminthic drugs are available at very low cost, and that school teachers can deliver treatment without the assistance of health staff; 1. URGES Member States: (1) to revitalize high-level political commitment and link malaria control services with economic growth and poverty reduction consistent with Roll Back Malaria approaches; (2) to ensure that health sector reform builds up capacity to control malaria in the periphery, maintains capacity for guidance, training, planning, evaluation and operational research at the national level, and that investment in malaria control is adequate, especially where the malaria problem is most severe; (3) to cooperate internationally on malaria surveillance and on confronting such problems as substandard and counterfeit antimalarial drugs and multidrug-resistant falciparum malaria; (4) to mobilize communities to take an active part in ensuring high levels of coverage with mass drug distribution campaigns for the elimination of lymphatic filariasis and other helminthiases; (5) to ensure that an increasing number of children have access to semi-annual deworming, in order to reach the target set by the World Health Assembly of covering 75%-100% of the school-age population by 2005; (6) to adopt national policies to integrate soil-transmitted helminth control activities into routine school health programmes; (7) to mobilize funds for parasitic disease control from national and local sources and, if necessary, from other sources such as health sector partners, the private sector and, for malaria, from the Global Fund to Fight AIDS, Tuberculosis and Malaria; 2. REQUESTS the Regional Director: (1) to ensure that available information on technical innovations and experience of controlling parasitic diseases, including malaria, lymphatic filariasis and soil- transmitted helminths, is shared with Member States; 14 REGIONAL COMMIITEE: FIFTY-THIRD SESSION (2) to continue to support countries in the planning, implementation and evaluation of programmes to control or eliminate parasitic diseases; (3) to facilitate coordination of partner activities and mobilization of funds; (4) to further strengthen regional surveillance of malaria and to promote information exchange on malaria control, antimalarial drug resistance, insecticide resistance, antimalarial drug quality, and the elimination oflymphatic filariasis; (5) to continue to advocate the regular deworming of school-aged children as an essential element of school-based health programmes. Eighth meeting, 20 September 2002 WPRlRC53/SRl8 WPRlRC53.R5 ANTIMICROBIAL RESISTANCE The Regional Committee, Recalling resolutions WHA51.17 on antimicrobial resistance and WHA54.14 on global health security; Concerned that prevalence of resistance to most common antimicrobials is growing at an alarming rate throughout the P..egion; Recognizing that antimicrobial resistance poses a threat to global and national health security; Noting that the emergence and spread of resistance are driven by numerous interconnected factors associated with misuse and overuse of antimicrobials in human and food-producing animals; Recognizing that the Global Strategy for Containment of Antimicrobial Resistance provides a framework that can be used to develop regional and national strategies; I. URGES Member States: (I) to develop and implement multisectoral strategies for containment of antimicrobial resistance, including strengthened national and local surveillance; (2) to adopt and enforce laws and regulations to ensure that antimicrobials be made available only on prescription; (3) to promote and ensure rational use of drugs, including antimicrobials, and to develop consumer education strategies; 2. REQUESTS the Regional Director: (I) to develop a regional strategy for containment of antimicrobial resistance; REPORT OF THE REGIONAL COMMITTEE 15 (2) to provide further technical support to Member States for the development and implementation of national strategies for containment of antimicrobial resistance; (3) to monitor prevalence of antimicrobial resistance in the Region and to share such information with Member States. Eighth meeting, 20 September 2002 WPRlRC53/SRl8 WPRlRC53.R6 ETHICAL ISSUES RELATED TO NEW DEVELOPMENTS IN THE HEALTH SECTOR The Regional Committee, Noting the continuing relevance of primary health care and the need to continue to adapt it in response to changing circumstances; Further noting the need to ensure that new developments in the health sector result in health care that is consistent with ethical principles, taking into account cultural and national contexts; Acknowledging that there are a number of key ethical concepts used in health care, research and other concerns related to health and development, including fairness in the allocation of resources, principles of biomedical ethics, equity, human rights, and consideration of the social and economic determinants of health; Recognizing that new developments in the areas of genomics, care of chronic conditions, patient safety and the integration of traditional medicine with modern medicine pose challenges for the health sector, some entirely new; I. URGES Member States: (I) to continue to use the primary health care concept, adapted in response to changing circumstances and specific country needs, as one of the bases on which health systems are built; (2) to ensure that principles of biomedical ethics, fairness, equity, human rights and consideration of social determinants of health are fully taken account of in health policies; (3) to build capacity to incorporate ethical principles and concepts into health- related work; 2. REQUESTS the Regional Director: (I) to continue to improve channels of information between Member States and WHO so that information on ethical issues can be shared; (2) to work with countries to encourage the integration of principles of equity and fairness into health policies; 16 REGIONAL COMMITIEE: FIFTY-TIlIRD SESSION (3) to ensure that the development of WHO programmes is firmly grounded on ethical principles; (4) to work with countries and WHO Headquarters to pursue further work in this area, to draw up a systematic framework to guide countries on integrating ethical issues into their public health policies, and to report back to the Regional Committee by 2006. Eighth meeting, 20 September 2002 WPRlRC53/SRl8 WPRlRC53.R7 ESSENTIAL PUBLIC HEALTH FUNCTIONS The Regional Committee, Having considered the report on essential public health functions: the role of ministries ofhealth;1 Acknowledging the need to ensure that the public health infrastructure is positioned to respond effectively and efficiently to broadening public health demands and challenges; Noting that the Western Pacific Regional Office has identified a need to evaluate and strengthen public health infrastructures in Member States in order to meet current and future public health demands; Recognizing that many essential public health functions are central to the responsibility of the State, particularly the ministry of heath, for effective stewardship with respect to health; Further recognizing the need for health sector reforms to pay greater attention to public health; Recalling that health systems were one of the eleven priorities identified in the general programme of work, 2002-2005, approved by the Fifty-fourth World Health Assembly in resolution WHA54.1, and recognizing that public health is a significant and crucial part of every health system; 1. URGES Member States: (I) to evaluate and monitor the delivery of all essential public health functions; (2) to strengthen their public health infrastructure wherever gaps and weaknesses are identified; (3) to incorporate work on essential public health functions into sectoral reform; (4) to monitor and evaluate the impact of policies of other sectors on public health and to respond in an appropriate and timely manner in order to ensure protection of public health; J Document WPRlRC53/1O. REQUESTS the Regional Director: (1) to pursue further consultation and discussions with Member States and other relevant parties on the proposed essential public health functions; (2) to develop guidelines, tools and indicators based on essential public health functions that will assist Member States to evaluate, monitor, and strengthen their public health infrastructures and responsibilities; (3) to promote the reorientation of health professionals, managers, policy-makers and government institutions in the Region towards public health, in line with the development of essential public health functions, and the strengthening of the central role of the ministry of health; (4) to disseminate to Member States the documentation on the three-country case- study on essential public health functions undertaken in the Region; (5) to report back to the Regional Committee by 2006.
