SALMONELLA SURVEILLANCE S p a in . — An outbreak of Salmonella enteritidis infection asso ciated with cheese occurred in Spain in November 1976. The first eight cases were diagnosed in the province of Albacete by the Provincial Health Services. The cases were in members of several families having no connexion one with the other. Symptoms consisted of vomiting, diarrhoea and fever, which appeared 8-20 hours after ingestion of a specific brand of cheese. The cause of the infection having been established, sale of the brand of cheese in question was banned. However, the product had already been distributed simultaneously in the provinces of Alicante, Albacete, Cuenca, Castellôû and Jaén, and a total of 1425 cases were reported.
SURVEILLANCE DES SALMONELLA E s p a g n e . — Une poussée de Salmonella enteritidis, liée à la consommation de fromage, a été observée en Espagne en novembre 1976. Les huit premiers cas ont été diagnostiqués par les services sanitaires provinciaux de la province d ’Albacete. Les personnes atteintes appartenaient à plusieurs familles n ’ayant aucun lien les unes avec les autres. Les symptômes (vomissements, diarrhée et fièvre) sont apparus de 8 à 20 heures après la consommation d’une certaine marque de fromage. La cause de l'infection ayant été établie, la vente de la marque en question a été interdite. Cependant, ce produit avait déjà été mis sur le marché, simultanément d a n s les provinces d ’Alicante, Albacete, Cuenca, Castellôn et Jaén, et, au total, 1425 cas ont été signalés. Les malades ont été traités à la tétracycline et au cbloramphénicol et tous ont réagi de façon satisfaisante, à l'exception d ’un enfant de sept ans, qui est mort quelques heures après son admission à l’hôpital. Les autorités sanitaires ont immédiatement fermé l’usine fabri quant le fromage, inspecté les locaux, contrôlé le procédé de fabri cation et examiné les manipulateurs de denrées alimentaires.
The patients were treated with tetracycline and chloramphenicol and all responded favourably, with the exception of a seven-yearold child who died a few hours after being admitted into hospital. The action taken by the health authorities consisted in the immediate closure of the factory producing the cheese, investigation of the premises and inspection of the manufacturing process, as well as the examination of foodhandlers. It was found that a fault existed in the method of pasteurization and that three of the foodhandlers bad suffered from diarrhoea a few days prior to the outbreak. In bacteriological examination o f faeces the same strain of S. enteritidis was isolated from both patients and foodhandlers. This same strain was also identified in the cheese, in addition to faecal streptococci, coliforms of the enterobacter o r citrobacter type and faecal Escherichia coli. The concentration of S . enteritidis in the cheese was in the order of KF/gr. The high degree of contamination of the cheese explains why many persons fell ill after ingesting only a small quantity taken as an appetizer.
L’enquête a révélé que la méthode de pasteurisation présentait un défaut et que trois manipulateurs avaient souffert de diarrhée quelques jours avant la poussée de salmonellose. Lors de l’examen bactériologique des selles, la même souche de S. enteritidis a été isolée chez les malades et chez les manipulateurs. Cette même souche a été identifiée dans le fromage, associée à des streptocoques fécaux, à des colifonnes de type enterobacter ou citrobacter et à Escherichia coli fécal. La concentration de S. enteri tidis dans le fromage était de Tordre de 105 /gr. L ’ampleur de la contamination du fromage explique pourquoi un grand nombre de personnes sont tombées malades après en avoir absorbé une faible quantité seulement à l’apéritif. Bien que le fromage ait été rare ment mis en cause dans les infections d ’origine alimentaire, sa fabri cation et sa manipulation nécessitent un contrôle sanitaire. E d it o r ia l N o t e : A particular food item should not be over looked as a source of foodborne disease simply because it has not previously been known as a common cause of gastro-enteritis. Although cheese has rarely been recognized as a source of foodborne infection, its manufacture and handling do warrant health control.
