SHIGELLA SURVEILLANCE S in g a p o r e . — Shigellosis is endemic in Singapore. The mean number o f admissions for bacillary dysentery at a major hospital between the years 1968 and 1975 was 63.6 (Table 1). No deaths have occurred since 1972.

The predominant shigella species isolated in Singapore in the years 1968-1975 were Sh.ftexneri (55.5%) and Sh. sonnei (41.6%). The numbers of Sh. dysenteriae and Sh. boydii were relatively insignificant, accounting for 0.89% and 0.69% respectively. Sh. sonnei emerged as the dominant species only in the years 1970,1971 and 1973. It is probable that Sh. sonnei would have been more prevalent if persons with milder diarrhoeal episodes sought medical treatment and had their stools cultured. In 1975, o f 51 cases and five carriers admitted to the hospital, 29 were males and 27 females. The five carriers (three Sh. sonnei and two Sh. flexneri) were detected from the screening of 3 857 foodhandlers from various eating establishments. Of the 51 cases admitted, five were imported and 12 came from a mental hospital. The onset of symptoms for most of the cases was during the first five months of the year with a peak in April. Cases were reported sporadically except for the small Sh. sonnei and Sh. flexneri out­ breaks at the mental hospital in February and March. Person to person transmission rather than infected food or water was res­ ponsible for the outbreaks. The median duration of hospitalization was two weeks (range 5-30 days).

Antibiotic sensitivity tests were carried out on 49 strains of Sh. flexneri and 12 strains of Sh. sonnei at the Department of Pathology. It was found that 76% of the strains o f Sh. flexneri and nine of the 12 strains of Sh. sonnei were resistant to triple sulphonamide. However, Sh. flexneri was still sensitive to the following antibiotics: ampicillin (94% of the strains), tetracycline (88%), chloramphenicol (96 %), trimethoprim-sulfamethoxazole (98 %) and neomycin (100%); and for Sh. sonnei, ampicillin (11 sensitive strains), tetracycline (ten), chloramphenicol (11), neomycin (1) and trimethoprim-sulfa­ methoxazole (12).

Les espèces de shigella isolées le plus souvent à Singapour pendant la période 1968-1975 ont été Sh. flexneri (55,5%) et Sh. sonnei (41,6%). Les dysenteries à Sh. dysenteriae et à Sh. boydii, très peu nombreuses, représentent respectivement 0,89% et 0,69% du total. Sh. sonnei n ’a été prédominante qu’en 1970,1971 et 1973. La fré­ quence de Sh. sonnei aurait probablement été plus élevée si les per­ sonnes atteintes de diarrhées bénignes avaient consulté un médecin et si les coprocultures de leurs selles avaient été faites. En 1975, sur les 51 cas et les cinq porteurs asymptomatiques hospitalisés, 29 étaient de sexe masculin et 27 de sexe féminin. Les cinq porteurs asymptomatiques (dont trois de Sh. sonnei et deux de Sh. flexneri) ont été découverts au cours d ’opérations de dépistage portant sur 3 857 manipulateurs d ’aliments dans divers établisse­ ments de restauration. Sur les 51 cas hospitalisés, cinq étaient importés et 12 provenaient d ’un hôpital psychiatrique. Le plus sou­ vent, les symptômes sont apparus au cours des cinq premiers mois de l’année, avec un sommet en avril. Les cas ont été signalés de façon sporadique, mises à part les petites épidémies à Sh. sonnei et Sh. flexneri de l’hôpital psychiatrique en février et mars. Au cours de ces épidémies la transmission s’est faite de personne à personne plutôt que par les aliments ou par l’eau. La médiane de la durée d ’hospitali­ sation a été de deux semaines (avec des valeurs extrêmes de cinq et 30 jours). Des épreuves de sensibilité aux antibiotiques ont été pratiquées sur 49 souches de Sh. flexneri et 12 souches de Sh. sonnei. D est apparu que 76% des souches de Sh. flexneri et neuf des 12 souches de Sh. sonnei étaient résistantes au sulfamide triple habituel. D ’autre part, Sh. flexneri était encore sensible aux antibiotiques suivants: ampi­ cilline (94% des souches), tétracycline (88%), chloramphénicol (96%), triméthoprim-sulfaméthoxazole (98%) et néomycine (100%); pour Sh. sonnei, les résultats étaient les suivants: ampicilline (11 souches sensibles), tétracycline (dix), chloramphénicol (11), néo­ mycine (12) et triméthoprim-sulfaméthoxazole (12).
